Glaucoma is a group of eye conditions that damage the optic nerve, which carries visual information from the eye to the brain. It is one of the leading causes of irreversible vision loss worldwide. In many cases, glaucoma develops gradually and may not cause noticeable symptoms until vision has already been affected. 

Early diagnosis and timely treatment are important because vision loss caused by glaucoma cannot be reversed. However, with proper management, further damage can often be slowed or prevented. 

Glaucoma occurs when the optic nerve becomes damaged, most commonly due to increased pressure inside the eye, known as intraocular pressure (IOP). In some cases, optic nerve damage can occur even when eye pressure remains within the normal range. 

Since glaucoma often progresses silently, regular eye examinations play an important role in detecting the condition before significant vision loss occurs.

Glaucoma is often symptom-less in early stages. But as it progresses, patients may notice:

  •  Patchy or missing areas in peripheral vision
  •  Difficulty adjusting to low light 
  • Blurred vision
  •  In-advanced stages: tunnel vision 
  • Acute angle-closure glaucoma may cause sudden eye pain, redness, halos around lights, and severe headache- this is a medical emergency.

Primary Open-Angle Glaucoma (POAG) 

The most common form of glaucoma. It develops gradually when the eye drainage system becomes less efficient, causing a slow increase in eye pressure and progressive optic nerve damage. 

Angle-Closure Glaucoma 

Occurs when the drainage angle becomes blocked, leading to a rapid rise in eye pressure. This condition can develop suddenly and requires urgent medical treatment. 

Normal-Tension Glaucoma 

Optic nerve damage occurs despite eye pressure remaining within the normal range. Reduced blood supply to the optic nerve may play a role. 

Secondary Glaucoma 

Develops as a result of another eye condition, injury, inflammation, steroid use, or certain medical conditions that affect the eye.

How is it Diagnosed

Glaucoma is diagnosed through a comprehensive eye examination. Tests may include: 

  • Eye pressure measurement (Tonometry) 
  • Optic nerve evaluation 
  • Visual field testing 
  • Optical Coherence Tomography (OCT) scan to assess nerve fiber thickness 
  • Gonioscopy to examine the drainage angle of the eye 

These tests help detect glaucoma, assess the extent of optic nerve damage, and guide treatment decisions.

Treatments Options

Glaucoma cannot be cured or reversed, but treatment can help control eye pressure and slow further damage to the optic nerve. The most appropriate treatment depends on the type and severity of glaucoma. 

Medicated Eye Drops 

Prescription eye drops are commonly used to lower eye pressure. They work by reducing fluid production inside the eye or improving fluid drainage. 

Oral Medications 

In some cases, oral medications may be prescribed to help reduce eye pressure, particularly when additional pressure control is needed. 

Laser Treatment 

Laser procedures can improve fluid drainage or create alternative pathways for fluid flow, helping lower eye pressure. The type of laser treatment recommended depends on the form of glaucoma being treated. 

Glaucoma Surgery 

Surgery may be recommended when glaucoma cannot be adequately controlled with medications or laser treatment. Surgical procedures help improve fluid drainage and reduce eye pressure, lowering the risk of further optic nerve damage and vision loss.

Who Is at Risk? 

Anyone can develop glaucoma, but certain factors increase the risk: 

  • Age over 40 years 
  • Family history of glaucoma 
  • Diabetes or hypertension 
  • High myopia (severe short-sightedness) 
  • Long-term steroid use 
  • Previous eye injury 
  • Thin corneas 

Living With Glaucoma 

Glaucoma is a long-term condition that requires ongoing monitoring and treatment. Regular follow-up appointments, consistent use of prescribed medications, and routine eye examinations are important for protecting vision. 

Because glaucoma often develops without noticeable symptoms, individuals with risk factors should undergo regular eye screenings, even if their vision appears normal.

Can glaucoma be cured?

No. Glaucoma cannot be cured, and vision loss caused by glaucoma is usually permanent. However, with early diagnosis and appropriate treatment, the condition can often be controlled. Treatment options such as prescription eye drops, laser procedures, and surgery are aimed at lowering eye pressure and reducing the risk of further damage to the optic nerve, helping preserve remaining vision.
