Glaucoma Surgery for Long Term Vision Protection

Glaucoma is one of the leading causes of irreversible blindness worldwide- and what makes it especially concerning is that it often develops without warning. Many patients don’t experience noticeable symptoms until significant vision loss has already occurred. That’s why understanding glaucoma and detecting it early is so important.

What Is Glaucoma?

 Glaucoma refers to a group of eye conditions that damage the optic nerve- the structure responsible for carrying visual information from the eye to the brain. This damage is most commonly caused by increased pressure inside the eye, known as intraocular pressure (IOP). However, glaucoma can also occur even when eye pressure is normal, depending on the individual’s optic nerve health.

Glaucoma Symptoms

Glaucoma is often symptom-less in early stages. But as it progresses, patients may notice:

  •  Patchy or missing areas in peripheral vision
  •  Difficulty adjusting to low light 
  • Blurred vision
  •  In-advanced stages: tunnel vision 
  • Acute angle-closure glaucoma may cause sudden eye pain, redness, halos around lights, and severe headache- this is a medical emergency.

Primary Open-Angle Glaucoma (POAG)- The most common type. It progresses slowly and painlessly, making it difficult to detect without routine eye exams. The drainage system of the eye becomes less efficient, gradually increasing pressure.

Angle-Closure Glaucoma- Less common but more urgent. The drainage angle becomes blocked, causing a sudden rise in eye pressure. This can lead to symptoms such as eye pain, headache, blurred vision, and nausea- requiring immediate treatment.

Normal-Tension Glaucoma- Optic nerve damage occurs even though eye pressure remains within the normal range. Often linked to poor blood flow or nerve sensitivity.

Secondary Glaucoma- Occurs due to another condition- injury, inflammation, steroid use, or certain eye diseases

How is it Diagnosed

Early detection relies on a comprehensive eye exam, which may include:

  • Eye pressure measurement (tonometry)
  • Optic nerve evaluation
  • Visual field testing
  • OCT scan to measure nerve fibre thickness
  • Gonioscopy to check the drainage angle

Treatments Options

Glaucoma cannot be cured, but it can be controlled. Early, consistent treatment helps prevent further vision loss.

Eye Drops- Medicated drops are usually the first treatment. They work by lowering eye pressure either by reducing fluid production or improving fluid drainage.

Oral Medications- Used in certain cases to rapidly reduce eye pressure.

Laser Treatments

  • SLT(Selective Laser Trabeculoplasty):Enhances drainage and reduces pressure- often used for open-angle glaucoma.
  • LPI(Laser Peripheral Iridotomy): Creates a tiny opening in the iris to prevent or treat angle-closure glaucoma.

Minimally Invasive Glaucoma Surgery (MIGS)- Modern procedures like iStent, Hydrus, or Kahook Dual Blade improve drainage with quicker recovery and fewer complications.

Traditional Surgery- Trabeculectomy or tube shunt surgery may be recommended for advanced or uncontrolled cases

Glaucoma can affect anyone, but certain factors increase the risk:

  • Age over 40 Family history of glaucoma
  • Diabetes or hypertension
  • Highmyopia (short-sightedness)
  • Long-term steroid use
  • Previous eye trauma
  • Thincorneas
